Clinical Lead Nurse – Adur Community PCN

We are seeking an experienced and qualified Community Specialist Practitioner with advanced practice credentials, strong clinical expertise, and proven leadership skills.

In this role, you will coordinate complex care, including the assessment and management of long-term conditions. You will apply case management principles to support patient self-management, promote independent living, and reduce hospital admissions through robust contingency planning.

As Clinical Lead, you will oversee staff management, supervision, and development, including appraisals and mentoring of junior team members. Acting as an educator and preceptor, you will lead the professional development of community nurses and champion self-care strategies for patients through innovative activation approaches.
