Clinical Lead Speech and Language Therapy for CNRT - 29 hours per week

This is an exciting opportunity to be the Clinical Lead for Speech and Language therapy within CNRT. You will devise, coordinateand implement service delivery.


You will work within the multidisciplinary team and be active in the development and promotion of integrated neuro rehab services in the community.


You will act as a specialist Speech and Language therapist within the field of neurological rehabilitation and will offer specialist assessments, treatment and management plans to patients with complex neurological conditions.

You will act as an expert source of knowledge providing education and advice to the patient, families and carers as well as the Multidisciplinary Team and wider community services. 


• To provide an advanced specialist service for CNRT, a specialist neuro-rehabilitation team for adults with acquired neurological disorders, in a community setting. 

• To work as a member of a team, providing specialist assessment, diagnosis and treatment for patients with varied and complex communication and swallowing difficulties. 

• Carries a personal caseload of adult clients and delivers a Specialist SLT service as part of an integrated multi-disciplinary team. 

• Provides supervision and advice in own specialist area to other therapists, assistants and students. 

• Has professional specialist knowledge and skills in adult neurology acquired through postgraduate SLT training, experience and increased depth and breadth of clinical competence.

• SLT qualification (degree level or equivalent, recognised by the Royal College of Speech and Language Therapists) and licence to practice as a SLT evidenced by registration with the Health and Care Professions Council (HCPC) 


• To be the named SLT lead for CNRT and to devise and implement appropriate clinical guidelines and patient pathways associated with this area. 

• To demonstrate Specialist Knowledge in clinical specialism, e.g. working knowledge of cognitive neuropsychology in language disorders, underpinned by current evidence based practice. 

• To perform advanced therapeutic assessment of patients with diverse presentations and complex communication and swallowing problems, to provide a diagnosis and develop and deliver an individualised treatment programme. 

• To be responsible for your own caseload and a defined area of the service, and provide support/supervision/training to colleagues working in your specialist area. Supervision takes the form of regular formal training and clinical reasoning sessions, peer review and case conferences.  

• To undertake all aspects of clinical duties as an autonomous practitioner. 

• Supervise, educate and assess the performance of students (this would be to a graduate standard and involve working with universities to ensure the standard of practice and teaching meets the standards set by the degree level qualification). 

• Undertake evidence-based audit and research projects to further own and team’s clinical practice within each rotation. Make recommendations to clinical team leader for changes to practice. May lead the implementation of specific changes to practice or contribute to service guidelines. 

• Evidence of a commitment to continuing postgraduate professional development (CPD) through a range of activities (individual study, attendance at short courses etc.).  Post holder to collect evidence of CPD by use of a professional portfolio and participation in the trusts personal development review process. 

• To access supervision and support (clinical and leadership) appropriate to the level of the role. 

• To participate in VFS and FEES as indicated. 





COMMUNCIATIONS 




• To communicate to clients and their families/carers specialist information about complex speech, communication and swallowing difficulties, to ensure they understand and consent to assessment procedures and can participate fully in decision making regarding management. 

• To use tact and empathy when dealing with sensitive issues surrounding patient care, e.g. when helping patient come to terms with the fact that his/her communication will be permanently impaired. 

• To apply highly developed listening and visual observation skills with clients and others in a range of settings, e.g. during structured one-to-one sessions, home visits, working both with direct observation and using audio and video recordings.  This will include tasks such as making phonetic and linguistic transcriptions, analysing speech and voice patterns, observations of social dynamics and non-verbal communication. Assessment of swallowing will require fine motor skills for palpation (feeling of laryngeal activity during swallowing). 

• To contribute to the multi-disciplinary team in discussion of own and other’s input around clients’ needs and management. 

• Use a variety of communication methods throughout the working day, eg: email, written records and telephone. 

• Ensure that all locally produced information documents are in the corporate style and that all written information complies with Trust policy. 






PLANNING AND ORGANISING  




• To organise own workload effectively, prioritising tasks and planning ahead by setting both long- and short-term SMART goals.  To plan and oversee work carried out by newly qualified SLTs, Technical Instructors and students. 

• To demonstrate reflective practice, reflecting on practice both independently and with peers; to demonstrate problem-solving skills in a peer-review/ support context and identify own areas of developmental needs. 

• To ensure attendance at all mandatory training and to attend regular staff meetings.  





 RESPONSIBILITIES 




• To manage a specialist caseload of adult clients including patients with communication, swallow and voice difficulties. 

• Carry full responsibility for assessment, diagnosis, treatment and discharge. 

• To assess and diagnose all in and out patients in general and own specialist caseloads. 

• To work collaboratively with the clients’ families/carers and other professionals, during assessment and all subsequent management, including decision making. 

• To communicate complex condition-related information from assessment to clients and their families/carers; this may include giving unwelcome or difficult to accept information e.g. around the seriousness, implications or long-term nature of the difficulty. 

• To provide specialist advice to patients, carers, nursing staff, Allied Health Professional (AHPs), medical staff and others regarding the management/treatment of patients with communication and/or swallowing difficulties. 

• To work in conjunction with team leaders but demonstrating skills to identify and deliver care plans for highly complex cases and generate and implement appropriate strategies for caseload management. 

• To provide written reports, presenting complex information in a clear and accessible way; reports may be provided to carers, GP’s, medical consultants and other allied health professionals. 

• To direct clients/carers to appropriate voluntary agencies and external services for support and information, e.g. The Stroke Association and other charitable groups. 

• To participate in the delivery and development of specialist training in many settings (e.g Nursing Home, CNRT department) to nursing staff, AHPs, carers.  Training will cover communication, dysphagia or both. 

• To take a lead role in developing and delivering specialist training (formal and informal) to others in area of clinical expertise. 


• To work with Technical Instructors/Speech and Language Therapy Assistant, providing programmes of therapy to be used with individual patients and supervising their implementation. 

• To provide training for staff within the department. To provide clinical training and supervision in own specialist areas to SLT students and junior/specialist  SLTs, other professionals, SLT Assistants and other students, e.g. student nurses.   

• To act as mentor for specialist SLT and junior staff; to provide advice and support to assistants and volunteers. 

• To maintain comprehensive, accurate and up-to-date case notes for own clients, adhering to professional and trust standards. 

• To undertake audit and data collection as required by the trust and submit timely and accurate data as required; this will include information about patient contacts, caseload figures and activity levels. 

• To be responsible for the line management of lower banding staff. 

• To be responsible for developing and monitoring clinical guidelines and patient pathways in your specialist area. 

• Embrace and support line management and the department, in making service improvements effective within the work place 

• To develop care protocols/packages relating to specialist area in liaison with Team Leader to improve patient care. 

• To contribute to interagency/multi-disciplinary team building and policy development. 

• This position requires the post holder to be able to travel between diverse clinical and non-clinical environments and bases, including patients homes, GP clinics, primary care facilities, hospitals, office spaces and residential/nursing homes, so access to reliable transport is essential. The post holder will be expected to complete an agreed number of clinical contacts a week, at separate addresses to maintain provision of our service when working within CNRT.
