Crynodeb
To support the delivery of the integrated working improvement and community development projects alongside our communities, VCSE and other NHS partners.
To plan, manage and deliver agreed projects within timescales and budget ensuring collaborative working across a broad range of stakeholders to support the delivery of the project objectives. As well as supporting integrated neighbourhoods this role aims to make operational change to improve cross-organisation working and remove the barriers to integration both internally and externally, alongside system partners.
The role requires close partnership working with clinical and operational leaders, the acute sector community sector, primary care, local authority, voluntary organisation, and patients to support the development of service redesign. It requires the ability to support the transition of a clinical vision into a coherent operational model of project delivery, ensuring service team tasks are identified, planned and executed effectively.
• Develop positive relationship building with integrated neighbourhood(s).
• Project lead for specified community projects associated with system change delivering specific placed based priorities.
• Project support to cross organisational meetings including co-production and presentation of highly complex and sometimes contentious information to support decision making, and design of agenda to enable key decision making to take place
• Support, facilitate and deliver joint working initiatives
• Support, facilitate and deliver integrated working improvement projects.
• Understand and interpret place-based health inequalities intelligence to maximise project delivery.
• Provide assurance projects are being delivered within accordance with the agreed plan of delivery.
