Assess own performance and take accountability for own actions, either directly or under supervision. Reviewing medical records and producing an accurate summary of the Patient’s medical history Participate in performance review and appraisals Sorting Patient records and letters in preparation for scanning/summarising Summarise and Read Code relevant information from clinical letters and patient’s clinical records into the Practice computer system (EMIS WEB), working strictly in accordance with the Practice notes & summarising protocol. Utilise relevant systems/equipment to be able to do the job to the best of your ability. Effectively manage own time, workload and resources. Timely management of clinical information received in practice. Work collaboratively with the multi-disciplinary general practice team to meet the needs of patients to support the delivery of policy and procedures. In the performance of the role, the post-holder will have access to confidential information relating to patients, carers, practice staff and other healthcare workers. They may also have access to information relating to the Practice as a business organisation. All such information from any source is to be regarded as strictly confidential. Information relating to patients, carers, colleagues, other healthcare workers or the business of the Practice may only be divulged to authorised persons in accordance with the Practice policies and procedures relating to confidentiality and the protection of personal and sensitive data.
