Crynodeb
The IBD Clinical Nurse Specialist (CNS) will provide a highly specialist nursing contribution to the clinical management and education of patients referred to the service in collaboration with the multidisciplinary IBD team across hospital and community settings.
• Acts as a clinical expert within the field of IBD medicine, and as a role model to others demonstrating high standards of quality care, facilitation and clinical leadership provision to others.
• Acts as a clinical expert in the development of clinical nursing services with the field of IBD medicine, implementing National strategies at a local level and contributing towards strategic decision making, and implementing policies appropriate to the speciality.
• Provides expert clinical advice and education to patients promoting a culture that supports self-management and health and wellbeing.
• Delivers specialist education and training to other professionals involved in patient care.
Leads the development, management and promotion of the service ensuring the delivery of high quality, cost-effective care.
Applies advanced knowledge and insight from key stakeholders and experiences of patients to lead services re-design, stimulate innovative practice, implement improvements and ensure services are responsive to patient’s needs.
• Promotes the philosophy of person-centred care, co-creating care with people affected by IBD.
• To provide an autonomous specialist nursing service to IBD patients by attending referrals and delivering holistic care to patients.
• Work in partnership with the patient and their family/carers to promote self-care and independence wherever possible through education and the use of self-management plans.
• Makes clinical judgements based upon experience and knowledge and takes appropriate action.
• Request diagnostic tests within agreed sphere of competence and make clinical judgements based upon clinical expertise.
• Uses advanced communication to alleviate the psychological distress in inflammatory bowel disease patients and their families, including identification of need for onward referral to other agencies as appropriate.
• Ability to motivate self and others with an Ability to work autonomously and as part of a team. Delegated responsibility for staff appraisal, managing attendance and performance capability
