JobSummary The Frailty GP role will require working on a rota with GP colleagues providing GP input to the following services within Bury: Ward 24 Hospital@Home - Virtual ward Intermediate Care Frailty SDEC - Same Day Emergency Care The role will involve being integrated within all 4 services. The nature of cover required by each individual service will mean at times working on-site with one service whilst being available for remote advice to other services. Frailty GPs will use their skills from General Practice to facilitate the best quality of care to frail patients and helping them to reach their full rehabilitation potential whilst also taking a holistic approach to anticipatory and End of Life Care. They will work towards the ethos of care for frail patients in their own place of residence where possible and minimise time away from that place when hospital care is unavoidable. GP Duties and Responsibilities Working autonomously as Lead Clinical Decision Maker when required for patients within the frailty services Working alongside Consultant Geriatricians and ACPs General medical care of patients including but not limited to: History and examination Investigation Diagnosis Treatment / care plans Therapeutic procedures Monitoring Holistic assessment and care planning for patients, including where appropriate anticipatory and end of life care Utilising available technology for remote communication and clinical assessment where appropriate Domiciliary visits where complexity/clinical need requires Communication with patients, their families and carers Communication with patients own GP when necessary Supporting junior medical staff Supporting all members of the MDT Actively participating in and when required/appropriate leading MDT meetings
