Crynodeb
This position provides the successful clinician with an opportunity to work directly within our community services and integral to the wider Oxon Place system to test, inform and finesse the development of the urgent community response programme, playing a key role in helping older people manage long-term conditions, making sure they receive the right kind of support to help them live longer and linking in with our healthcare colleagues in the wider community. You will be working alongside and supporting the Community Teams including the Community Therapy Service and District Nursing services however you will have the opportunity to work across many facets of Community services.
