Key activities of the post holder will be to: Act as a Specialist Pharmacist for Intestinal Rehabilitation and Nutrition Services including independent prescribing, nutrition ward rounds and outpatient clinics. Support the pharmacy Intestinal Rehabilitation team within the organisation and across interface boundaries. To establish and enhance an effective pharmacy link service between The Royal Marsden and St Georges Hospitals, improving continuity of care for intestinal failure patients. Work as an expert pharmacist for Intestinal Rehabilitation and Nutrition Services, contributing to advanced decision making within multi-disciplinary teams at St Georges Hospital and jointly with our partner Trusts. Manage and implement services designed to improve the pharmaceutical care to patients and extend the boundaries of clinical practice. Identify and support the facilitation of CIPs within the relevant directorate and participate in financial reporting and drug budget management. Progress service improvement initiatives through multi-disciplinary working to ensure safe and timely provision of medicines. To participate in service development and evaluation, audit, research, risk management and clinical guideline development, both within St. Georges and in conjunction with St. Marks in line with the West London Intestinal Rehabilitation Service. The successful applicant will enjoy working in a busy challenging environment, be able to manage multiple priorities and deliver against tight deadlines. You will have excellent communication skills and be an enthusiastic educator. The ability to work autonomously and with a high level of accountability is essential. They should possess a postgraduate qualification in clinical pharmacy or equivalent and have the relevant hospital pharmacy experience. Experience of working and influencing in a multi-disciplinary team is essential
