Crynodeb
Deliver evidence based nursing care across a wide range of clinical areas, including long term condition management, immunisations, cervical screening, wound care, health promotion, and infection prevention and control. Undertake patient assessments, provide routine and urgent nursing care within scope of practice, and offer advice to patients and their families on managing their conditions. Support population health approaches by contributing to public health campaigns, targeting health inequalities, maintaining disease registers, and using call/recall systems effectively. Work collaboratively with the multidisciplinary team and external agencies to ensure coordinated, high quality patient care. Participate in clinical audits, quality improvement initiatives, research projects, and the development or review of practice policies, protocols, and procedures. Maintain accurate clinical records, comply with national and local guidelines, and uphold standards within areas such as safeguarding, clinical governance, and infection control. Engage in ongoing training, clinical supervision, annual appraisal, and contribute to shared learning within the practice. Support everyday operational needs, including chaperoning, assisting with clinical emergencies, ensuring a clean working environment, and helping with rota coordination when required.
