Clinical Fellow - Frailty
This post will support the service to deliver a high standard of medical clinical care as part of a team of Specialty Doctors and Consultants. As part of a skilled multidisciplinary team, the post holder will be responsible for undertaking patient assessments and delivering treatment for frail & older patients attending the emergency department then transferred to the Frailty Same Day Emergency Care and Frailty Decisions Unit. The objective is to help these patients return to their normal place of residency at the earliest opportunity.
Main duties:
To participate in the Consultant led ward round (Frailty Decision Unit)
To identify patients in the Emergency Department suitable for Frailty SDEC.
To complete a Comprehensive Geriatric Assessment as part of the wider multi-disciplinary team, under the direction of the Consultant.
Ensure that medical care is provided to the patients in line with the agreed Medical Plan.
The appropriate discharge of patients, including providing timely paperwork to the General Practitioner ensuring a continuation of care following discharge.
Ensure that discharge plans are fully integrated into wider community health and social services support plan.
Under direct clinical supervision from the Speciality Doctors and Consultants ensure that F1/F2 doctors have a high standard of training and to participate in the Grand Rounds.
To participate in and perform audits as outlined by the Educational Supervisor.
Complying with data protection.
Complying with all relevant Trust Policies and Procedures.
Undertake to perform additional duties in occasional emergencies and unforeseen circumstances at the request of the appropriate Consultant / Specialty Doctor. Additional duties arising under this sub section will not persist for prolonged periods or be on a regular basis.
