Job Summary Bexley Health Neighbourhood Care(BHNC)is aGP Federation supporting the21Bexley Practices/ 4PCNs,Erith Hospital and Queen Marys Urgent Treatment Centrewho areworking collaboratively to enhance the health and wellbeing of Bexley residents, covering over 246,000 patients. The post holder willbe responsible forthe assessment of care needs and the development,implementationand evaluation of programmes of care within the Urgent Treatment Centre. Deliver excellent patient care through continuous review ofevidencedbased clinical standards for patients presenting with undifferentiated diagnosis with illness and or injury. Contribute to the quality, monitoring,evaluationand the development of the service. The post holder will be passionate about the patients inBexleyand beyond helping them to make better use of the NHS locally and supporting clinicians to make the best use of their skills. Organisational Values / Objectives The goals ofBHNCare to work strategically with all Bexley Primary Care Networks (PCNs), to help secure the best services for patients whilst working together, to support the member practices in the challenges of a changing NHS. Bexley Health Neighbourhood Care (BHNC) aims to improve the morale of PCNs / general practice/UTCsin Bexley, by sharingexpertise, services and supporting its workforce.BHNC will make a positive impact on medical services in Bexley, by working closely with the CCG, local NHS trusts, localprovidersand patient groups, to improve the delivery of healthcare to the local population. Key Working Relationships Key relationships include but are not limited to: BHNC Board BHNCSenior Management Team BHNC Training Hub UTC Senior Management Team Key Responsibilities Duties: Use holistic patient assessment skills of adults and children so that clinical decision making will ensure prompt andappropriate actionin the best interest of the patients/familieswell being. Undertake assessment of their health and presenting complaint, diagnosing, treating, givingadviceand discharging or referring patients to other health professionals including the GP and speciality on call teams according to protocols. Request andinterpret X-rays and emergency blood tests ifappropriate. Act as an effective role model, through the application of skills to patient care acting as advisor to the wider MDT. Work under pressure in a rapidly changing environment. Prescribe medications under own professional scope of practice and within the formulary acknowledging own professional boundaries. Keepaccurateand immediate records of every enquiry including assessment andadviseregardingpatient care using both Adastra, Cerner and EMIS Encourage patients to carry outself careasappropriate througheffective communication and health care advice. Be responsible and accountable for all the patients on own caseload, seeking guidance from the Clinical Lead GP when clinicallyappropriate. Maintain own professional development.Identifiesany training needs and attend relevant courses necessary to ensure best practice. Keep up to date with current developments in patient care andevidence basedpractice. Work within all policies,protocolsand guidelines of the UTC and BHNC. Ensure that confidentiality ismaintainedat all timesincluding the storage of records and the transfer of verbal and written information. Work within multidisciplinary care teams promotinggood communicationwith the Trust and hospital staff, statutory and voluntary agencies. Ensure that patients and carers who face cultural or language barriers are helped to understand and make use of the service available. Identifyany safeguarding issues patients may have and act on the information. Contribute to the recruitment and selection process. Support other members of the health care team. Ensurenew staffare familiar with and adhere toBHNCpoliciesand clinical pathways and policies. Take part in theday to daymanagement of the team at the UTC. Maintain effective communication with stakeholders and other outside agenciespromoting the image of the UTC at all times. Contribute to the development, monitoring,auditingand reviewing of policies, procedures protocols and guidelines within the UTC. Participate in an annual individual performance review, including taking responsibility formaintaina record of own personal and/or professional development Participate in any training programme implemented by the management team Effectively manage own time,workloadand resources Assess own performance and development and take accountability for own actions, either directly or under supervision. Maintain confidentiality at all times. Represent the organisation positively and professionally, behaving as an ambassador for BHNC bydisplaying the organisations values and levels of expected professionalism at all times. Demonstrate interpersonal skills when dealing with all levels of staff across the BHNC / UTCs using persuasion,tactand reassurance where necessary. Handle confidential patient material in a sensitive and discrete fashion in compliance with Bexley Health Neighbourhood Care policy and procedure. To be able to influence, motivate and involve individuals and teams to reach necessary performance target expectations. Liaise with other departments and members of the Bexley Health Neighbourhood Care and other organisations to ensure the smooth running of the role toassistingthe functioning of the UTC team. Represent the organisation positively and professionally, behaving as an ambassador for BHNC bydisplaying the organisations values and levels of expected professionalism at all times.
