Key Roles and ResponsibilitiesClinical Care Assess, diagnose and manage same-day urgent and acute presentations Provide home visits for acute presentations in housebound patients Carry out physical health reviews for dementia patients and respond to deterioration Request and interpret investigations within scope of practice Prescribe medicines where qualified Escalate to GPs or secondary care when required Decision-Making & Risk Work independently and manage clinical risk Make decisions in uncertain and complex situations Assess mental capacity and act in patients best interests Team Working Work closely with GPs and the wider MDT Take part in frailty and complex care MDT meetings Communicate clearly with patients, carers, and professionals Leadership & Service Support Provide clinical support and advice to colleagues Contribute to service improvement and undertake annual prescribing audit Support admission avoidance and care planning Professional Responsibilities Maintain accurate clinical records Work within NHS, NICE, and HCPC/NMC guidance Follow safeguarding and information governance policies Maintain professional development, attend supervision and keep a record of this
