Crynodeb
In conjunction with other team members, to take on the role of care coordinator for a designated caseload of clients with both organic and functional mental health problems. To be responsible, for the comprehensive assessment, planning, implementation of programmes of care, providing each client with an individual holistic package of care. To ensure Mental Health Measure and care management protocols are adhered to and standards are met. As care coordinator, responsibility for making evidence based assessment of clients physical, psychological, spiritual and social needs taking into account and respecting ethnicity, for a defined caseload.
