Crynodeb
The Practice Nurse is responsible for delivering high-quality, evidence-based nursing care to patients within primary care services across Barking & Dagenham. Working autonomously and as part of a multidisciplinary team, the post holder undertakes a wide range of clinical duties including health assessments, chronic disease management, immunisations, health screening, and health promotion. The role supports safe, effective service delivery in line with NMC standards, clinical guidelines, and Together First CIC policies, while contributing to service development, quality improvement, and the support of colleagues.
