Please see the attached Job Description and Person Specification To lead and provide high-quality clinical pharmacy services for Homecare and day-case specialties, ensuring the safe, effective and evidence-based supply, prescribing and monitoring of medicines for patients within these settings. The post-holder will participate in Consultant-led ward rounds when required and, where appropriate, practise as an independent non-medical prescriber to optimise patient care. They will maintain strong multidisciplinary relationships, ensuring clear, timely and effective communication across the wider MDT. They will identify opportunities for clinical development and service enhancement, presenting proposals to the DCSS Triumvirate management team and supporting the planning and implementation of changes that meet patient needs and maintain sustainable service income. The post-holder will implement local and national Homecare guidance in partnership with clinical teams, ensuring that policies, pathways and procedures are current, accessible and effectively communicated to the wider workforce. They will act as the pharmacy departments subject matter expert for Radiology and Endoscopy, supporting the wider clinical workforce in these areas. As governance lead for Radiology and Endoscopy, the post-holder will represent pharmacy at divisional governance meetings, take forward actions arising, manage risks, and ensure full compliance with medicines legislation and national standards. They will communicate complex and highly specialist pharmaceutical information to healthcare professionals, patients and carers, adapting information to individual needs to support safe treatment, informed decision-making and medication adherence The post-holder will lead on the creation and delivery of the clinical pharmacy audit programme, ensuring audit outcomes are translated into service improvements and best practice. They will also supervise, mentor and train pharmacists, pharmacy technicians on rotation, and other healthcare professionals working within the specialist areas, ensuring high professional standards and continuous development within the pharmacy workforce.
