Would you like to work as part of a dedicated Children in Care team within the wider 0-19 Service?
You would be responsible for assessing, planning and implementing holistic health care plans. This care may be delivered in schools, homes, clinics and other appropriate settings.
You would be undertaking statutory health assessments and promoting the health and wellbeing of children and young people.

Establish clear and effective communication links with young people, parents/carers, social care staff and a range of other professionals from both voluntary and statutory organisations.
Listen and take into account sensitive issues and communicate these to appropriate persons/agencies ensuring that children and young people are protected and that disclosures are acted upon appropriately.
To keep accurate and up to date electronic and written records, provide written reports as requested by the Service Leads.
Utilise a range of communication methods with colleagues and managers to include electronic communication.
Assess, plan and implement interventions and programmes of care for young people within the care system, up to the age of 25 yrs.
Identify the relevant health and social issues within the specific cohort of young people leaving care.
Recognise indicators of child and adult abuse and initiate Safeguarding measures in accordance with recognised policies, and procedures of CCS, and relevant Local Safeguarding Children Board, Safeguarding Adults Boards.
Demonstrate an evidence-based approach to practice to enable the development of a high standard of nursing practice.
Undertake visits in the community, some outside normal working hours, in accordance with CCS Lone Workers Policy.
Undertake health assessments of Children and Young People Leaving Care.

Participate in policy development to improve the service, and update guidelines and standard operating procedures when required.
Take appropriate action in cases of suspected or actual child abuse and work closely with Health Visitors/School Nurses and the Trust Safeguarding Team to ensure that children are protected from harm.
To be familiar with the Trusts Safeguarding Procedures and attend yearly safeguarding updates.
In accordance with the needs of the service, undertake any other duties as required by the Service Manager/ Named Nurse.
Adhere at all times to the NMC Code and Cambridgeshire Community Services NHS Trust Policies.
