Crynodeb
Main Duties of the Job: Supporting chronic disease management (e.g. asthma, COPD, hypertension, heart failure) Provision of childhood and adult immunisations. Promoting health education and self-care Providing travel health advice and immunisations Contributing to QOF, enhanced services, and practice development initiatives Work within the EMIS Web clinical system to record consultations, manage recalls, and contribute to clinical audits. Collaborate with GPs, pharmacists, practice nurses, HCAs and external services. Support the practice in meeting QOF, IIF, and other long-term condition targets. Provide ongoing education and lifestyle advice to empower patients in self-management. Support service development and audit quality improvement initiatives.
