Calderdale and Huddersfield NHS Foundation Trust are seeking an enthusiastic Heart Failure Specialist Nurse / Team Leader to join and lead the existing community Heart Failure team to deliver care to patients closer to home and preventing unnecessary hospital admission across Calderdale community.



The post holder will provide clinical and operational leadership for the community heart failure service, with a particular focus on the development, delivery, and oversight of the Heart Failure Virtual Ward.

As Team Lead, the role involves providing direct support to both the Cardiac Rehabilitation Lead and the Heart Failure Team in the strategic planning and operational management of the service. This includes, team management, staff development, service performance, quality improvement and collaborative leadership as well as holding a caseload of complex patients, working across the community, Virtual Wards, clinics, and remote monitoring pathways.

The key aims of the service are to provide high quality, evidence based, patient centred care

through a structured and pro-active approach. This will ensure the patients, and their relatives are

at the centre of decisions about care, ensure their voices are and those of their advocates are heard and their choices and priorities known and respected.

· Lead on the day-to-day clinical coordination of the Heart Failure Service managing referrals, patient flow, and escalation plans.

· Deputise for the Cardiac Rehabilitation lead, including chairing meetings, coordinating team activities, and managing service priorities.

· Operational oversight of the Heart Failure Service / virtual Ward, including patient triage, allocation of visits, and ensuring timely clinical interventions.

· Act as a key liaison between hospital and community teams to support seamless transfers of care into and out of the virtual ward.

· Contribute to strategic planning and service development, including business cases, pathway redesign, and integration with PCNs and ICB priorities.

· Ensure safe and effective remote monitoring of patients using digital platforms, physiological monitoring tools, and structured clinical reviews.

· Act as the clinical point of contact for primary care, acute trusts, and urgent care services referring to or discharging from the virtual ward.

· Participate in data collection and evaluation of virtual ward outcomes and innovations. To be responsible for ensuring that the induction process of new staff to the service area takes place.

· Monitor rate of readmission, identifying any trends and working within the multidisciplinary team to address these

· Work with all members of the multidisciplinary team to develop practice, research and audit.

· Work within the framework of clinical governance, work continuously to improve quality and safeguard standards of care, maintaining a patient focused service, promoting and

implementing evidence-based practice with measurable standards of care and outcomes.

· Be involved in audit projects to measure standards of care. To identify and highlight any

gaps in the evidence/service provision for Heart Failure patients.

· Critically examine working practices within the department to identify cost improvements and effective measures, develop and implement action plans

· Understand the commissioning of services and any income generation from new work or changes in service that may affect income

· Understand the procurement of services in terms of service development initiatives, lead the team through service changes.

· To participate and lead in the writing of departmental guidelines and implement Trust wide/departmental policies, procedures and guidelines.

· Maintain, advance and develop personal theoretical knowledge, high standards of clinical competence and professional development through on-going self-education and attendance at relevant courses of study.

· Develop and maintain a peer network of support, information and learning with other nurse specialists at a local and National level.

· Maintain own professional portfolio.
