Historically, there have been boundary lines between the organisations that provide care to people in their homes, in GP surgeries and in hospitals, but we have always been united in our mission to provide great care to the people who need us.



It’s on those grounds that the Surrey Downs Health and Care was formed – we want local people to receive the care that they need in the right environment. By bringing together our expertise, we can improve patient care and enable local people to access the right support, care and treatment more easily than ever before.

This role provides rehabilitation support within the Community Neuro Rehabilitation Service, delivering neuro-rehabilitation interventions in outpatient, home, and community settings under the guidance of qualified therapists. The rehab assistant supports treatment across Physiotherapy, Occupational Therapy, and Speech and Language Therapy, managing a routine caseload and assessing and treating patients within agreed guidelines. They monitor patient progress, making minor adjustments to treatment programmes as appropriate, and work independently on a day-to-day basis while reporting progress and any concerns to the treating therapist. Regular guidance is sought from therapists to ensure safe and effective practice.



· Record information accurately and timely.

To organise administrative tasks efficiently to support smooth and effective service delivery.

To engage in Continuing Professional Development (CPD) activities to maintain and enhance personal and professional competence.

· Attend all relevant in-service training sessions to support ongoing skills development.

· Participate fully in the SDHC appraisal process and developing a personal development plan aligned with the Knowledge and Skills Framework (KSF).
