We are seeking an enthusiastic Occupational Therapist with 1–2 years of NHS experience & prior involvement in stroke rehabilitation to join the Integrated Stroke Early Supported Discharge (ESD) and Community Neuro Rehabilitation Service (CNRS).

ESD is a key pathway for stroke survivors, allowing them to leave hospital earlier than they may have otherwise. CNRS supports stroke patients that do not meet the ESD criteria and other neurological conditions in the community.

This is an exciting opportunity to join an established multi-disciplinary team comprising Occupational Therapists, Physiotherapists, Speech & Language Therapists, Psychologists, Specialist Nurses & Rehab Assistants.

This role requires the candidate to manage a clinical caseload across the North of Cambridgeshire, their base will be Peterborough, Doddington, Wisbech or Ely. It also involves visiting patients in their homes, wards or community gyms. There may be times when cover is required elsewhere in Cambridgeshire along with agile working & using virtual consultation to manage a caseload.

You will provide comprehensive assessments and deliver evidence-based specialist neuro–occupational therapy programmes.

You will work as part of a multidisciplinary team in supporting patients with physical, cognitive, and psychosocial needs to achieve their potential. Given the specialist nature of this service, applicants must bring prior experience of stroke rehabilitation—whether gained in acute, community, ESD, or neurorehabilitation settings—to ensure confidence and competence when working with this patient group.

The ideal candidate should be flexible and adaptable to support service delivery. The successful post holder must be able to travel independently across their work locality, have a valid driving licence and access to a car for work purposes.

• To provide a high standard of Neuro Occupational Therapy, performing assessments of patients and to deliver an individual treatment programme.

• To provide ESD in-reach to the community beds for patients under the ESD service and work with patients in their own homes for both ESD and Neuro rehab patients.

• To hold responsibility for own caseload and be responsible for a defined area of the service working unsupervised within the multi-professional Integrated Stroke ESD and Community Neuro Rehabilitation Team.

• To be accountable for assessing, interpreting, planning, implementing, and evaluating treatment to patients under the care of the Integrated Stroke ESD and Community Neuro Rehabilitation Team within professional guidelines, with the support of Specialist and Advanced Practitioners within the team. Providing appropriate equipment and rehabilitation programmes and making referrals on to other professionals and agencies as appropriate in order to meet the identified needs.

• To identify patient needs, agree goals and provide appropriate therapy interventions relating to their physical, functional, and psychological needs.

• To be involved in providing 6 month reviews for stroke survivors.
