Summary
An excellent opportunity has arisen Triage Co-Ordinator/ Senior Therapy Assistant role in a friendly and supportive team within the BHFT. The successful candidate (s) will be working alongside an established multi-disciplinary team consisting of senior specialist community physiotherapists, Occupational Therapists, therapy assistants, and team administrators. As part of this role the post holder will be an integral member of the Clinical support.
Team. We are an integrated team including Occupational Therapists, Physiotherapists, Therapy assistants, and admin support. The service supports multiple pathways including Community Reablement, Falls and Community Physiotherapy. This is an exciting time to join BHFT, as we develop services to work in an integrated way within the community in West Berkshire and across Berkshire West. The successful candidate will be required to provide/ willing to develop skills in providing high quality, responsive services working as an integral member of the team. You will also work closely with social services and voluntary organization for adults with a wide range of physical impairments and mental health conditions.
• To be part of discussions regarding patients progress while on the service and review with the multi-disciplinary team as appropriate.
• To have a patient centred approach.
• To liaise with Registered Nurses, Registered Occupational Therapists, Registered Physiotherapists and Care Managers/Social Workers as required and to keep accurate and concise reports.
• To contribute and participate in professional development meetings.
• To be the first point of contact for patients and health and social care partners wishing to discuss referrals and to escalate accordingly as and when appropriate.
• To coordinate adequate therapy assistants coverage where needed.
• To undertake exercise visits as per clinician advice, supporting patient to progress and improve mobility and ADL’s.
• To complete initial assessments of patients on all pathways.

The “must haves” for you to be considered for this role:
• NVQ 4 or Equivalent experience.
• Experience of working with older people.
• Excellent communication skills
• Ability to Drive & have use of a car
