Job Description:

We currently have an exciting opportunity for a Support Officer - Young People's for our Housing Related Support service.

The service is designed for young people aged 18-25 who are leaving care, including young people at risk and those who may have committed an offence. We aim to provide specialist support to help young people to develop their independent living skills, sustain a tenancy, and integrate into community living.

The main purpose of the role is to:

To provide a programme of person-centred support to customers referred into the Young People HRS in accordance with contract requirements
To engage with young people on a weekly basis to support individuals to secure, maintain and achieve independent living in the community
The co-provision of support plans, risk management plans, crisis intervention and engaging with young people in order to deliver positive outcomes in a service that offers involvement and empowerment for customers
To provide a high-quality service; ensuring outstanding customer service and service delivery by acting as an ambassador for Humbercare and point of contact for customers.
Engaging with customers to design services that work for them and utilising resources within the local area to make a real, quality difference to peoples' lives

Salary - £26,208.00 per annum
