Spectrum have an exciting opportunity to join our team at HMP Holme House working as an Advanced Clinical Practitioner supporting our primary care team.
Advanced clinical practice is delivered by experienced, registered health and care practitioners. It is a level of practice characterised by a high degree of autonomy and complex decision making. This is underpinned by a master’s level award that encompasses the four pillars of clinical practice, leadership and management, education and research, with demonstration of core capabilities and area specific clinical competence using the multi-professional advanced clinical practice framework.
The Advanced Clinical Practitioner (ACP) will have the ability to manage clinical care in partnership with individuals, families and carers, where appropriate. This will include the analysis and synthesis of complex problems across a range of settings, enabling innovative solutions to enhance people’s experience and improve outcomes. ACPs are highly experienced and educated members of the care team who are able to diagnose and treat conditions and refer onwards to appropriate specialists if needed. As such the ACP within Health and Justice will be expected to:
•Take responsibility for own learning and development by recognising and taking advantage of all opportunities to learn, in line with core professional competencies of the ACP, including full participation in appraisal and supervision, and acting as role model for self-development
•Provide autonomous clinical assessment and treatment, demonstrating clinical expertise at an advanced level, in physical health with elements of mental health, substance misuse and learning disabilities for patients within Health and Justice.Contribute to the delivery of programmes of quality improvement within Health and Justice settings
•Provide advanced and expert level of practice (including non-medical prescribing) within Health and Justice, leading to improved patient outcomes
•Provide clinical leadership and management leading to the provision of safe, quality, effective care.
