Summary
Core Functions: Provide senior clinical consultancy and leadership across all urgent care services. Lead PSIRF-aligned governance, investigations, and learning processes to improve safety culture. Promote workforce transformation and ACP professional development. Line manages the Practitioner workforce across Directorates. Drive service innovation and the implementation of evidence-based models of care. Support the Directors and Associate Directors to deliver strategic objectives. Will demonstrate the knowledge and skills of a competent consultant practitioner and will: Autonomously manage the complete clinical care for patients, to enhance patient care and improve patient pathways in line with national standards. Autonomously undertake comprehensive physical and mental health assessments, providing differential diagnoses and deciding on the appropriate treatment based on clinical findings for patients with complex multi-dimensional acute or chronic clinical conditions. Support admission avoidance and remote management of patients where possible dependent on service type with appropriate conversion to face to face dependent upon clinical need. Completing onward referrals as appropriate to primary secondary and third sector organisations. Prescribe medication independently as appropriate, select and order investigations independently, refer to other professionals independently for tests or examinations autonomously within the scope of agreed clinical guidelines. Provide consultant clinical leadership, educational and clinical supervision to other staff within the area of responsibility, ensuring that there is provision of the relevant clinical advice, guidance, and support to undertake duties. The consultant clinical practitioner will evidence the four domains of the consultant level framework across all aspects of the patient pathway. Administrative Responsibilities Ensure that medical and patient records are comprehensive, accurate, and up to date. Complete relevant coding forms accurately, providing a clear explanation of the patients condition and treatment plan. Ensure the proper completion of ReSPECT forms and advanced care planning, tailored to the patients level of frailty, clinical condition, co-morbidities and wishes. Accurately document all discussions regarding care, ensuring that conversations with the patient and family members are properly recorded and discussed as appropriate. Clinical Responsibilities Work autonomously at consultant level to assess, diagnose, plan, and evaluate complex patient care across urgent care environments. Maintain approximately 45% clinical commitment, providing senior clinical oversight and modelling excellence in advanced practice. Oversee complex case management and escalation processes. Ensure evidence-based clinical practice, safe prescribing, and appropriate investigations and interventions. Support clinical safety, quality assurance, and event management in line with PSIRF principles. Autonomously, assess, differentially diagnose, plan, implement, and evaluate individual treatment plans, interventions, and care packages for patients, ensuring safe and appropriate management, timely referrals, and safety netting as required. Prescribe independently, review medication for therapeutic effectiveness, and ensure treatment is tailored to patient needs, adhering to contraindications, drug interactions, evidence-based practice, national and practice protocols, and within scope of practice to optimise outcomes. Maintain effective communication within the MDT and with wider stakeholders, ensuring active participation with senior team members and facilitating clear communication across roles. Communicate independently with external stakeholders and agencies to ensure continuous, high-quality care during both admission and discharge. Support patients and families receiving "bad news," engaging the team as needed to provide appropriate emotional support. Act as an advocate for patients and colleagues, ensuring needs and concerns are effectively represented. Collaborate with clinical teams to improve healthcare quality, population health responding to local and national policies and initiatives and assessing impact on service delivery. Partner with the Associate Director of Governance and quality compliance to develop and implement the workforce strategy for advanced practice at Nimbuscare. Responsible for assessing ongoing care needs and planning effective transitions, ensuring continuity and quality of care. Ensure clear communication of care plans to the team, fostering collaboration and consistency. Operate diagnostic equipment to monitor and assess patients, ensuring accurate readings for informed clinical decisions. Management and Leadership Responsibilities Line manages the Practitioner workforce across Directorates Provide clinical leadership in line with the Patient Safety Incident Response Framework, ensuring clear alignment with company values, strategic direction, and policies impacting care delivery Maintain up-to-date knowledge in the specialist field, using this information to drive change in practice and ensuring leadership in the effective dissemination of new knowledge. Possess the theoretical and practical knowledge necessary to deliver job responsibilities to a high standard. Act as a visible clinical leader, meet standards, and improve patient outcomes and family/carer satisfaction. Provide clinical supervision to staff, supporting and inspiring colleagues to improve standards, quality of care, and professional practice and demonstrate competency. Lead as a change agent and innovator, planning, instigating, and evaluating change as part of a continuous improvement cycle Develop policies, to support clinical care and practice within the specialism, ensuring the effective implementation of policies across the service. Manage and prioritise own workload according to patient needs and service requirements. Represent the team at appropriate meetings. Participate in the recruitment and retention of staff. Contribute to the effective management of resources and the safe use of equipment. Ensure the delivery of one-to-ones, cascade information to all staff, and provide forums for objective setting, performance reviews, and continuing professional development. Works in collaboration with the senior leadership team to investigate events. Policy and Service Development Lead service development efforts, writing protocols, guidelines, and procedures within your remit that also impact other members of the multi-disciplinary team. Develop and evaluate protocols, guidelines, and procedures based on current literature and research. Contribute to the development of patient information to support patient care. Implement policies and communicate any changes to the multi-professional team. Actively contribute to the clinical governance agenda, ensuring compliance with statutory and mandatory training. Support the development of advanced and enhanced clinical practice. Lead and deliver quality assurance programmes, assessing risk and ensuring that adequate measures are in place to safeguard patients, staff, and families/carers. Participate fully in Practice Development Group meetings, clinical supervision, and other key meetings. Research and Audit Responsibilities Lead audits and research within your clinical area. Influence the strategic development of advanced practice research and development at local, regional, and national levels. Utilise research findings to inform and enhance clinical practice. Education and Training Distribute tasks effectively among the team, ensuring an even workload and providing guidance to ensure high standards of care and professionalism. Utilise professional knowledge and skills, underpinned by theoretical and practical experience, to teach, motivate, and support junior staff: Provide structured teaching and practical support to junior staff, fostering a learning environment to help them build clinical skills and confidence. Oversee performance appraisals and support professional revalidation, ensuring staff meet regulatory standards and receive guidance for ongoing development. Develop and implement educational programmes for patients and carers, providing essential information on treatment plans and care procedures to promote engagement and understanding. Work with the Associate Director of Urgent Care, Leads of Service, Advanced Practice Lead, General Practitioners, and other Nurse Consultants to provide clinical guidance and educational support to junior staff, ensuring continuous learning and high-quality care. Lead training sessions for healthcare workers, enhancing skills and knowledge to improve care delivery across Nimbuscare services. Conduct teaching sessions for multidisciplinary teams to raise awareness and ensure coordinated, high-quality care for the patient group. Supervise and mentor students during clinical placements, providing constructive feedback and supporting professional growth.
