Crynodeb
The post-holder is a New to General Practice Nurse. A New to General Practice Nurse provides fundamental nurse-led care to communities across the PCN. They support public health and screening programmes including immunisations and vaccination programmes while participating in quality improvement initiatives to enhance patient outcomes and reduce health inequalities. The New to General Practice Nurse undertakes the early detection, prevention, and management of cardiovascular disease and other long-term conditions. This post-holder is expected to take a lead role in managing specific long-term conditions following structured mentorship and preceptorship and work within a multi-disciplinary team, supporting integrated care and personalised approaches to patient management. This role will take a lead in the following areas: Dementia Reviews and Support Mental Health Reviews and Care Learning Disability Reviews and Support Hypertension and Lipid Patient Care Assist GP with Care Home care provision including weekly ward rounds The post-holder will be an integral part of the general practice team, working in each practice and across the PCN, as well as participating in the wider practice nurse network and multi-disciplinary team. They are to participate in formal training events and to promote best practice in their area of expertise. When a PCN employs or engages one or more New to General Practice Nurse under the Additional Roles Reimbursement Scheme, the PCN must ensure that each has the key responsibilities in delivering health services, as outlined in Annex B (section B21) of the Network Contract Agreement DES Specification PCN Requirements and Entitlements 2025/26.
