Crynodeb
The collection and interpretation of comprehensive and accurate data for the Integrated Discharge Team. Manage data and information to assist the Integrated Discharge Team in sustaining the work of the team, which will contribute to the overall improvement of Discharge Services across the whole Organisation. Extraction of data from PAS, ICE, Optica and SystmOne Monitor the Discharge Hub Outlook Box keeping up to date with correspondence within the integrated Discharge Team in a timely manner. Process referrals from current referral system (ICE at time of writing) and accurately record and manage referred patients Optica entries. Complete all elements of the covid Discharge Situation Report daily and send to NHS England. Ensuring the Patient Tracker spreadsheets are up to date: daily use entries in addition to totalling referral and discharge tables and Fast Track Outcome sheets Completing the daily Discharge Summaries. Support with Out of Area Referral forms, liaising with Out of Area Trusts and Social services as required. Respond to ad-hoc requests from senior nursing and medical staff for information related to quality and improvement schemes within the Integrated Discharge Team.
