Description
The Family Support Worker - Grade 4 position is a pivotal role within our organization, dedicated to providing essential support and guidance to families in need. The Family Support Worker will also be responsible for maintaining accurate documentation of interactions, outcomes, and progress, ensuring that all support provided aligns with best practices and organizational protocols. You will participate in team meetings and case reviews, contributing your insights and working together with colleagues to enhance service delivery. Responsibilities
Provide direct support and guidance to families facing challenges.
Conduct assessments to identify family needs and strengths.
Develop individualized support plans in collaboration with families.
Facilitate access to community resources and services such as housing, education, and mental health support.
Monitor and evaluate family progress towards goals, adjusting plans as necessary.
Offer parenting education and support to enhance family functioning.
Maintain accurate and confidential case records and reports.
Requirements
Bachelor's degree in social work, psychology, or related field preferred.
Experience working with families and children in a social service setting.
Strong interpersonal and communication skills.
Ability to work collaboratively within a team and with external partners.
Cultural competence and sensitivity to diverse family dynamics.
Excellent problem-solving skills and the ability to manage difficult situations.
Familiarity with community resources and services available to families.
