Key Task and Responsibilities Use advanced clinical skills for assessing and treating patients with diabetes using relevant clinical protocols for newly diagnosed patients with diabetes, patients with unstable diabetes and patients with complex needs and/ or complications of diabetes who are referred to District Nursing Service. To be professionally and legally responsible and accountable for all professional activities. To be able to work alone in the community in a variety of settings, independent of multidisciplinary support whilst in direct contact with patients and their carers. To provide line management where required a to Nurses/Associate Nurses within the Community Diabetes Team, including supervision, performance management and appraisal. To contribute to the planning and development of the diabetes service in Bexley in accordance with National guidance. To participate in diabetes education programmes to meet the needs of people in the local population diagnosed with Type 2 Diabetes both as one-to-one or group sessions. To work in a co-ordinated manner, communicating effectively with other team members and the wider clinical network involved in patient care. To maintain up-to-date clinical written records and electronic database in accordance with local and professional standards. To be able to identify when and how to terminate involvement or refer onwards to appropriate teams/services and complete input in a timely, seamless manner. To seek guidance from more experienced colleagues when required. Capability to deputise for senior colleagues where appropriate.
