Crynodeb

We are seeking passionate and skilled Early Intervention Support Workers to join the Hampshire Neurodiversity and Wellbeing Multidisciplinary Team (ND MDT). This innovative service is part of a system-wide transformation to move from a diagnosis-led approach to a needs-led model, ensuring children and young people with emerging neurodivergent traits, and their families, receive timely, holistic support.

As an Early Intervention Support Worker, you will play a key role in delivering the “family support” function of the MDT, helping families navigate services, access resources, and implement strategies that improve daily life and school experiences. You will also work closely with schools to promote inclusive practices and ensure that education settings are equipped to meet the needs of neurodivergent learners.

The Early Intervention Support Worker will have a work bases in the MDT’s district hubs; some travel to visit local families, and on occasion schools in the district, a requirement of the role.

There are six roles/locations available, and you will be asked to state your preference on your application form. The roles are temporary until March 2027, but with the possibility to extend.
- Havant and East Hants District
- New Forest and South Test Valley
- Basingstoke and Dean and North Test Valley
- Winchester and Eastleigh
- Hart, Rushmoor and East Hants
- Fareham and Gosport

What you’ll do:
- Provide support and signposting to families and/or professionals concerned about a child’s potential neurodivergence (including autism and ADHD).
- Act as a link between families, schools, and professionals, ensuring clear communication and coordinated care.
- Support schools and other practitioners in using the Neurodiversity Profiling Tool, helping them understand the child’s strengths and needs.
- Facilitate access to peer support groups, helplines, and digital resources.
- Work collaboratively with schools to embed inclusive strategies, improve attendance, and reduce exclusions.
- Contribute to family-centred plans and attend MDT panel reviews where appropriate.
- Promote early intervention strategies and reduce reliance on formal diagnosis for accessing support.
- Provide flexible support across all age ranges (0–19), including primary, when required, to ensure continuity and equity of service.

What we’re looking for:
A proactive and organised professional with experience supporting children, young people, and families—ideally in neurodivergence or SEND contexts. You’ll bring strong communication skills, knowledge of neurodevelopmental conditions, and a commitment to inclusion, working collaboratively across agencies to improve outcomes for neurodivergent children and their families.
