The post holder will act as an experienced physiotherapist, helping to develop integrated health care teams in the communitywith ina Multi-Disciplinary Team (MDT). The post holder will provide high quality physiotherapy interventions to a range of patients from different community settingssuch as:frailty, rapid response / admission avoidance, intermediate care, falls prevention through to preventative action and support for self-management oflong termconditions. The caseload will include advanced assessment and treatment of patients, provision of individual and group education and treatment, provision of assistive devices (e.g.orthotics) and equipment as well as liaising withothersservices andagencies.



Main duties and responsibilities

• To manage a defined caseload using evidence based /client centred principles to assess, plan, implement and evaluate interventions,maintainingassociated records.

• To provide a high standard physiotherapy service to the specialist caseload across various sites within Tower Hamlet as the service requires.

• To perform advanced physiotherapeutic assessment of patients with diverse presentations and complex physical and psychological conditions to provide a diagnosis and develop and deliver an individualized treatment program.

• To provide leadership for more junior staff, as agreed with senior staff.

• To deputies for the Senior Physiotherapists in their absence asrequired.

• To supervise Physiotherapy students on clinical placements.

• Toparticipatein CPD and develop skills and knowledge through participation in thein servicetraining scheme.

• To undertake lone working whenrequired.

• To be able to work flexibly andparticipatein potential future 7 day working developments (whenrequired)

Clinical Responsibilities

• To be professionally and legally be accountable for all aspects of own work, including the management of patients in your care

• To undertake all aspects of clinical duties as an autonomous practitioner

• Tobe responsible formaintainingaccurateand comprehensive patient treatment records in line with legal requirements and Health & Care Professions Council (HCPC)standards ofpractice

• To understand the legal framework around gaining informed consent for treatment and to implementappropriate proceduresin line with trust policies

• To have a comprehensive understanding ofpatients’clinical presentations based upon advanced clinical knowledge

• To develop specialist knowledge ofparticular conditionsand patient presentations specific to different diagnoses and treatments

• To undertake a comprehensive subjective and objective assessment of patients with diverse or complex presentation or multiple pathologies anduse advanced clinical reasoning skillsto accuratelyidentifyimpairments,abilitiesand participation

• To developtimelyand comprehensive discharge plans, consideringappropriate futureinterventionin accordance withprognosis

• To assess clinical risk within own patient caseload and to be aware of risks involved in the caseloads of other team members

• To formulate, plan, deliver,evaluateand progress a patient-centredphysiotherapy treatment and management program by effective and efficient clinical reasoning, using currentevidence basedpractice (e.g. Best Practice Statements, journal review, in-service training, post-graduate training, peer review)

• To use valid and reliable outcome measures to evaluate patient progress, reassessing and adjusting treatment programs as indicated

• To assess and treatappropriate patientson a weekend day. This will include patients needing not only treatment for respiratory issues but also patients who require rehabilitation andlong termmanagement.

• To work closely with the multi-disciplinary team, acknowledging areas ofuni- and inter-Professionalexpertise, ensuringoptimalout-come for both patients and service

• To be an active multi-disciplinary team member,ensuring a coordinated patient-centredapproach to service delivery. This will include multi-disciplinary team discussion of patient care, prognosis, goalattainmentandtimelydischarge planning

• To developtimelyand comprehensive discharge plans, consideringappropriate futureinterventionin accordance withprognosis

• To liaise with, and provide advice and education to, medical, nursing staff, social services staff and other AHPsregardingthe 24-hour management of patientson a daily basisand in all emergency duty situations

• Toidentifychange in patient presentation and arrange for urgent or non-urgent review by other disciplines

• Tobe responsible forsafe and competent use of all equipment, appliances and aids by both patients and staff under your supervision

• To develop specialist knowledge ofparticular conditionsand patient presentations asencounteredthrough working in the community health teams.





Professional Responsibilities

• Toadhere to HCPC/ CSP Codes of Conduct and Standards of Practice at all timesensuring that those working under your supervision do the same

• To positively promote the role of Physiotherapy

• To work within national and Trust (East London Foundation Trust) clinical guidelines,policiesand standards and CSP guidelines. Monitor own and others quality of practice, ensuring safe working practices with reference to these. Have a basicunderstanding of the Vulnerable Adults policy and Child Protection Act.

• To consistentlymaintainbest practice, Continuing Professional Development and Career Pathway through, for example, attendance at postgraduate courses, in- service training, journal club and Special Interest Groups, encouraging staff under your supervision do likewise.

• Maintainan up-to-date diary reflecting CPD activities and personal and professional development. To have agreed protected time in which to complete this

• To take responsibility forparticipatingin PDP and meeting setobjectives.

• To beparticipateactively in in-service training programs, tutorials, Clinical Governance days, peer review and courses, both internal and external

• Toparticipatein, and/or lead departmental and multi-disciplinary audit and research initiatives into specific areas of clinical practiceutilizingsupport and supervision from senior staff asrequired.

• To undertake evidence-based audit and/or research projects to further own and team’s clinical practice within each rotation

• To present findings to the therapy and wider team and be involved in resultant service development and changes to practice

• To provide clinical support and supervision to assistants and qualified physiotherapists working at a lower gradeon a daily basis

• To attend necessary clinical educator and appraisal training

• To lead and/or contribute to the performance assessment and appraisal of qualified physiotherapists working at a lower grade and physiotherapy assistant staffon a daily basis, including taking the role of Appraiser at PDP meetings

• To supervise, educate and assess the performance of physiotherapy students to a graduate standard on a regular basis. To work in conjunction with universities to ensure the standard of practice and teaching meets the standards set by the degree level qualification

• To be able to provide formal teaching/training to other multi-disciplinary team members ifrequested, and to teach basic skills/knowledge on an informal basis throughout day-to-day practice

• To provide learning/observational opportunities to a variety of medically linked students e.g. Physios’,OccupationalTherapists, Medics, and Nurses, on an individually agreed basis





Organisational

• Tobe responsible fororganizing and planning owncase-loadto meet service and patient priorities, allowing adequate time for non-clinical duties, such as report writing, leasing within the team, across services and other agencies, and student supervision

• To deputies for the Band 7 in their absence on an agreed, short-term basis, taking responsibility for operational management of the team (juniors, assistants,studentsand any A&C staff),allocatingand organizing thework-loadto meet service priorities

• To prioritize theteam work-load as temporary staffing shortages arise, e.g.short termsick leave

• To be flexible during the working day, allowing response to unexpected situations, e.g. acute patient illness, urgent referrals

• Toassistin the induction of more junior staff members and students

• Tobe responsible forthe supervision and co-ordination of junior staff,studentsand assistantson a daily basis

• To be actively involved in the instigation, organization and running of both in-service training and external courses

• To be involved in the care and maintenance of equipment as per manufacturers guidelines and relevant policies, including the identification and reporting of faults and problems





General

• The CHTs if required by the population of Tower Hamlets, work over extended days (8am – 10pm) including weekends and the post holder would need to be able to work occasional shifts within these times as required by the CHTs.

• Toadhere to the Trusts incident-reporting policies and procedures at all times, co- operating with investigations undertaken in respect of this.

• To be able toidentifythe need for, andparticipatein, flexible working.

• Tocomply withall Trust policies including equal opportunities and manual handling.

• Tocomply withthe requirements of the NHS and Social Care related legislation including the Data protection Act 1994.

• To carry out such duties as may be required by the professional/line manager consistent with the responsibilities of the grade.

• To attend mandatory training and induction courses asdesignatedby the Trust.

• To abide by HCPC rules of professional conduct and local professional and quality standards.
