Description
We are seeking a dedicated and compassionate Complex Needs Navigator (Level 6) to join our dynamic team. This role is pivotal in connecting individuals with multifaceted needs to the appropriate services and support within our community. As a Complex Needs Navigator, you will work directly with clients who may be experiencing challenges related to mental health, housing, substance use, and other social determinants of health. Your primary focus will be to assess client needs and facilitate a comprehensive, coordinated approach to service delivery that empowers and enables individuals to achieve their goals. You will collaborate with a variety of stakeholders, including healthcare providers, social services, and community organizations to create individualized care plans tailored to each client’s unique situation. The ideal candidate will have a strong background in case management, advocacy, and a thorough understanding of the social service landscape. Your ability to navigate complex systems and effectively communicate with diverse populations will be essential in this role. Responsibilities
Conduct comprehensive assessments of clients' complex needs and develop individualized service plans.
Provide ongoing support and guidance to clients throughout their journey, ensuring they access appropriate resources and services.
Collaborate with healthcare providers, social services, and community organizations to ensure a seamless service delivery system for clients.
Advocate for clients’ rights and needs within various systems and institutions to ensure they receive adequate support.
Monitor and evaluate clients’ progress, making adjustments to care plans as necessary to meet evolving needs.
Educate clients and their families about available services and resources, empowering them to make informed decisions.
Document client interactions, case notes, and service outcomes in accordance with organizational standards and compliance regulations.
Requirements
Bachelor's degree in social work, psychology, sociology, or a related field; a Master's degree is preferred.
Demonstrated experience in case management, social work, or community services with a focus on complex needs populations.
Strong understanding of mental health, addiction, and housing issues, and the related service systems.
Exceptional interpersonal and communication skills, with the ability to engage with diverse clients.
Proven advocacy skills, with a commitment to empowering clients and being their voice.
Ability to work independently and as part of a multidisciplinary team to achieve shared objectives.
Strong organizational skills, including the ability to manage multiple cases and meet deadlines.
Hours Per Week: 36.00
Start Time: 09:00
End Time: 17:30
Pay Per Hour: £18.42
