Crynodeb
You will work as part of a compassionate, multidisciplinary team, working in partnership with the patient, family & carers. You will: Assess, plan and deliver holistic, person-centred care. Support patients and carers physically, emotionally and spiritually. Keep accurate records and communicate clearly with the wider care team. Use your knowledge of local services to ensure the best support for every family. Occasionally provide cross-site cover when needed.
