The role will require managing and prioritising your own caseload, in accordance with the needs, priorities and any urgent support required by individuals on the caseload. This is a sample of responsibilities expected from the Social Prescriber, please see the Job description for full details. Take a holistic approach, based on the patients priorities and the wider determinants of health. Working in partnership with all local agencies to raise awareness of social prescribing in the Network and the benefits to services and health outcomes in can bring Forge strong links with local VCSE organisations and community and neighbourhood groups. Work with these groups to ensure directory of community assets and support reflects the services available in St Helens North network. Be proactive in encouraging self-referrals and connecting with all local communities, particularly those communities that statutory agencies may find hard to reach. Actively promote social prescribing services within the practices Support practice staff with social prescribing referral processes Train practice staff to identify patients suitable for social prescribing by identifying the most at-risk patients in need of proactive care Keep in contact with the patients at agreed intervals Takes responsibility for personal development, learning and performance Act in a way that recognises the importance of peoples rights, interpreting them in a way that is consistent with Practices procedures and policies, and current legislation. Respect the privacy, dignity, needs and beliefs of patients, carers and colleagues. Behave in a manner which is welcoming to and of the individual, is non-judgmental and respects their circumstances, feeling priorities and rights. Recognize peoples needs for alternative methods of communication and actively reduce communication barriers
