Summary
Key responsibilities and Duties: 1. Surgery Consultations, triaging calls as appropriate, occasional home visiting and administration associated with patient care. 2. Work as a team member with all relevant providers of primary and community care services within the practice. 3. Ensure appropriate records are maintained at all times regarding patients care and that relevant team members are kept fully informed regarding patients seen on behalf of the practice. 5. Ensure that the practice achieves the highest possible QOF outcomes. 6. Developing care plans for health in consultation with patients and in line with current practice disease management protocols. 7. Support the proactive care agenda by participating in care planning and multi-disciplinary meetings. 8. Attend team and healthcare meetings when required. Other responsibilities in the practice. 1. Maintain confidentiality of the practice at all times. 2. Work in accordance with and comply with all practice policies and procedures/guidelines. E.g. health and safety, prescribing, GDPR, and assist in their development 3. Fulfil any monitoring requirements attached to the post as required. 4. Maintain and develop individual skills and knowledge, ensuring statutory, mandatory and Trust required training updates are completed in a timely manner. Committed to lifelong learning. 5.Attend training and events organised by the practice or external agencies where appropriate. Actively participate in clinical governance within the practice.
