Crynodeb
This role provides prompt, efficient coordination and administration support to a multi-disciplinary team (MDT) within the Health and Social Care setting. Working with relevant MDT professionals to support implementation of care provision and tracking progress of care. Ensures a prompt response to crisis referrals and co-ordination ofinterventions by team members or ensuring referrals are made promptly toother services to prevent unnecessary hospital admission. To maintain the information systems used by the team in order to collate and co-ordinate performance data and produce reports as required for the team and relevant managers.
