Summary


· To work as part of a multi-disciplinary team and working with key workers in the team to support patients experiencing their First Episode of Psychosis.

The role will include working with the patients support network.

The role includes driving to patient’s homes, a good level of rapport building and patient engagement and enabling patients to feel at ease.

To ensure that the highest standards of care are maintained at all times.

To ensure that high standards of record keeping are an integral part of this role.

To liaise with relevant agencies which can include, Primary Care, safeguarding, community mental health teams, inpatient services as required.



To maintain up to date training and standard IT skills, demonstrating a good understanding of confidentiality and data protection.

To accurately maintain contemporaneous clinical records and documentation ensuring accuracy and confidentiality are maintained at all times.

Report concerns to key worker/duty worker/team leader.

To have an awareness of issues surrounding confidentiality and data protection.

This is a fixed term role and for those with current NHS service, the role will be offered on a secondment basis only. Please obtain permission to be released on a secondment before submitting an application.
