Description
We are seeking a dedicated and proactive Referral Officer (SO1) to join our dynamic team. In this role, the successful candidate will be responsible for overseeing and managing the referral process for individuals requiring various services and support. As a key point of contact, the Referral Officer will facilitate communication between clients, service providers, and various stakeholders to ensure that individuals receive timely and appropriate assistance. This includes assessing the needs of clients, coordinating referrals, and following up to ensure successful service engagement. The ideal candidate will possess excellent interpersonal skills and a deep understanding of the local service landscape. They will work diligently to maintain productive relationships with community organizations and service providers, enhancing our referral network to better serve our client base. By ensuring that clients are connected with the right resources, the Referral Officer plays a vital role in contributing to the overall mission of improving client outcomes and fostering a collaborative environment. The successful candidate will also engage in data tracking and reporting, providing insights into referral trends and service utilization to inform program development and enhance strategic planning efforts.
Responsibilities
Manage the referral process for clients accessing services.
Assess and identify client needs to determine appropriate referral resources.
Coordinate with service providers to facilitate timely referrals and follow-ups.
Maintain accurate records of referrals and client interactions.
Develop and sustain relationships with community organizations and service providers.
Provide clients with information about available services and options.
Monitor the effectiveness of referrals and report on outcomes.
Requirements
Bachelor's degree in social work, psychology, or a related field.
Proven experience in case management or similar roles.
Strong understanding of community resources and referral processes.
Excellent communication and interpersonal skills.
Ability to work independently and as part of a team.
Proficiency in data entry and management systems.
Strong organizational skills and attention to detail.
Hours Per Week: 17.50
Start Time: 09:00
End Time: 17:00
