Summary
Consultant Practitioner with clinical expertise in frailty? We are seeking an expert clinician to lead on a number of system priorities, including Frailty Hospital at Home, and clinical support with the development of Integrated Neighbourhood Teams.
This is an exciting time to join as we further develop our Frailty Hospital at Home. in partnership with our colleagues in University Hospitals Dorset, with a ‘one team’ approach and the post holder will work in partnership with acute clinician’s and teams to deliver the flagship service across the Bournemouth, Christchurch and Poole area.
You will be established and confident with respect to complex clinical reasoning with a holistic approach. Be comfortable with a high degree of clinical risk taking whilst thinking outside of the box. You will have a passion to help influence and shape the future direction of the role and service. This is an exciting scope of work with a variable and developing pace of change.
The Trust continues is committed to the development of Advanced Practice, with an expectation that the post holder will provide clinical leadership.
This Consultant Practitioner post is a full-time role.. The role will be based in Bournemouth and Poole and you will be required to work flexibly to meet the needs of the service across the conurbation.

You will work collaboratively with the multi-professional team, practice autonomously, whilst being accountable and self-directed in line with the relevant code of professional conduct. There is an expectation a minimum of 50% of your portfolio will be spent in direct autonomous clinical practice, providing clinical input and/or clinical consultative sessions within a defined cohort of patients, service users and providing leadership for clinicians within this clinical area.
To lead as a driver for change, to monitor and improve standards of care, safeguarding and clinical effectiveness through supervision, evidence-based practice, clinical audit, national and international research, and education and support for professional colleagues.
Maintain and develop a local and national profile as a Consultant Practitioner being a creative and innovative leader in developing services for the relevant client group.
Advise and communicate as appropriate with acute hospitals, primary and social care and community teams ensuring seamless continuity and transfer (if appropriate) of care for patients between other health, social and third sector agencies, professionals and other care settings.
Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.
