A Care Coordinator supports the management team to ensure the effective and successful running of the service. Working directly with the people receiving care and support and their families. They’ll liaise with other healthcare professionals, such as local authorities and the Care Quality Commission.

You primary job is to support the Home Care Manager in running the service to ensure success.

What does a Care Coordinator do?

A Care Coordinator job with Papa Home Care comes with a range of responsibilities. Some of the general responsibilities and duties of a Care Coordinator include the following:

- Administration – updating & maintaining digital records
- Communicating – discussing purchasing fees with local authorities & with privately funded clients
- Compliance – investigating complaints & taking the appropriate remedial action
- Coordinating – changes to visiting times & assessing the needs of people looking for care & support
- Customer Service – providing great customer service at all times
- Developing Business – accepting new packages of care from a variety of different referral routes
- Delivering Care & Support – providing care & being available for out of hours on-call services
- Scheduling – preparing staff rotas & reorganising when necessary
- Supporting – carrying out supervisions & assessments; identifying staff training needs.

What will you need to do the job?

A good Care Coordinator will have:

Excellent communication skills as they’ll be working with a variety of different people within their own company and externally.

Exceptional organisational abilities and the confidence to handle several tasks at once. You need to be reliable and able to build trusting relationships and be a great role model for other members of the Team.

You will need a driving licence and access to a vehicle with business insurance. You will need to travel around the local area to visit clients in their own homes to carry out assessments or deliver care. You may also need to transport clients to hospital appointments or on shopping trips.

You will need professional experience in providing care and support. Ideally in a Home Care setting. But we welcome applicants with other similar experience.
