Specific Duties: Work as part of a multi-disciplinary team within the PCN Consult with patients, their families and the wider team to undertake assessments of patient need and devise and evaluate complex care plans Evaluate and analyse clinical problems using clinical knowledge, applying evidence, enhanced techniques, interventions, and equipment Deliver enhanced clinical care in a context of continual change, innovation, and evolving technologies Teach and advise patients and families on managing their conditions or support the MDT to do so Participate in clinical audits, research projects, and implement required changes Develop and update practice protocols and local guidelines Work within national and local protocols where applicable Recognise boundaries of practice and refer patients appropriately Delegate work appropriately within the MDT using the principles of delegation Demonstrate initiative and creativity in solving problems Hold responsibility for team performance and service delivery Provide supervision to trainee nursing associates, nursing associates and general practice nurses Wider Responsibilities: Facilitate and support the development and delivery of innovative training and education to improve staff competence and confidence Lead and/or participate in formal training events promoting best practice Provide professional and clinical leadership and mentorship, acting as a role model for evidence-based practice Support the development and delivery of PCN work streams aligned to areas of clinical expertise Work proactively with stakeholders to develop integrated care pathways and holistic models of care Contribute to local policy, procedures and protocols to improve patient and carer support General Duties: Support achievement of agreed targets and participate in regular supervision Engage in training and ongoing professional development Maintain confidentiality at all times Cover work of colleagues during absence as necessary Support compliance with CQC standards and internal policies Work flexibly, including occasional out-of-hours work where required Health and Safety: Promote and maintain their own and others health, safety and security Manage risks effectively Engage in mandatory training Maintain tidy and safe work environments Report any potential risks identified Equality and Diversity: Support the equality, diversity and rights of colleagues, patients and carers Behave in a manner that respects individuals' dignity and rights Personal / Professional Development: Participate in annual appraisals and maintain professional development records Maintain NMC registration and adhere to codes of conduct Manage own time, workload and resources effectively Undertake clinical supervision Participate positively in education and team meetings Use clinical systems accurately and consistently Develop and review clinical protocols Quality: Promote a culture of quality improvement Engage in clinical audit and reflective practice Recognise and work within personal competence Deliver care according to evidence-based standards Support shared learning and risk management initiatives Team working Act as an effective team member Delegate clearly and appropriately Manage and prioritise own workload Support new service delivery models to improve patient care Manage or lead specific projects as agreed with management
