Crynodeb
Assess, diagnose, treat and where appropriate refer or signpost patients presenting with undifferentiated or undiagnosed conditions, within scope of practice. provide same day and planned home visits, including urgent assessments and follow up. Support care homes with acute visits, proactive rounds and multidisciplinary meetings. Request, perform and interpret appropriate investigations such as ECGs and blood tests, in line with local guidance. Contribute to anticipatory and personalised care planning, including for people with long-term conditions and frailty. Promote self-care and health improvement, including signposting to social prescribing and community services. Maintain accurate, timely clinical records in the practice clinical system and contribute to audit, governance and quality improvement activity. Work collaboratively with the wider MDT, attending relevant clinical and PCN meetings. Maintain professional standards in line with HCPC requirements and local policies, including active engagement in supervision and CPD. Support the delivery of QOF, incentive schemes, QIPP and other quality effectiveness initiatives Undertake any tasks consistent with the level of the post and the scope of the role, ensuring that work is delivered in a timely and effective manner
