Crynodeb
The post holder will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines management, the delivery of Structured Medication Reviews and management of long term conditions and minor ailments. Providing clinical leadership on medicines optimisation and quality improvement, the post holder will also manage some aspects of the quality and outcomes framework and enhanced services. The development of systems to effectively manage medicines upon transfer of care into primary care or care homes is also a requirement of the post. The post holder will work to develop systems for safer prescribing and share innovations across the PCN practices, as well as the provision of clinical pharmacist functions at a practice level. The post holder will be an experienced Clinical Pharmacist, who acts within their professional boundaries to support the development and mentoring of the pharmacy team. The post holder will hold an independent prescribing qualification or is working towards the qualification. The post holder will be encouraged to specialise in an area of focus aligned to delivering the PCN priorities.
