      • Provide health promotion, disease prevention, and lifestyle advice.

      • Empower patients in self-management of long-term conditions.

      • Communicate effectively with patients, families, and carers.

        Teamwork & Collaboration
        • Work closely with GPs, nurses, paramedics, pharmacists, and administrative teams.

        • Maintain accurate, timely electronic patient records.

        • Contribute to multidisciplinary meetings, case discussions, and integrated care pathways.

          • Strong clinical reasoning, decision-making, and diagnostic skills.

          • Excellent communication, leadership, and organisational abilities.

          • Ability to manage complex, unpredictable caseloads in a fast-paced environment.

            Desirable Experience

            • Experience in primary care or urgent treatment centres.

            • Familiarity with EMIS Web, SystmOne, or similar clinical systems.
