Summary
Job Summary We are seeking a compassionate and motivated Band 5 Social Prescribing Link Worker to join our multidisciplinary team. This role is pivotal in supporting people with long-term health conditions, as well as children and families, to improve their wellbeing through non-clinical, community-based support. As a Link Worker, you will work in partnership with primary care professionals, local authorities, schools, and voluntary/community sector organisations to empower individuals to take greater control of their health and wellbeing. You will address the wider social determinants of health by connecting people with a range of services including community groups, parenting support, physical activity opportunities, mental health resources, welfare advice, and more. Key Responsibilities Provide personalised 1:1 and group support to individuals referred to the service, helping them to identify their goals, develop a wellbeing plan, and connect with appropriate community resources. Work with adults living with long-term physical and/or mental health conditions to improve self-management, resilience, and independence. Support children and families, including those experiencing social isolation, low-level mental health issues, or socio-economic challenges, to access appropriate local services and improve wellbeing. Collaborate with GPs, health visitors, school nurses, community paediatrics, social care, and local voluntary organisations to ensure a coordinated, holistic approach. Use motivational interviewing, coaching, and strength-based approaches to empower service users to make positive changes in their lives. Maintain accurate and up-to-date records of all interactions and interventions using approved systems, contributing to service evaluation and quality improvement. Build and maintain strong knowledge of local services, voluntary groups, and social opportunities to ensure effective signposting. Monitor and follow up with service users to assess progress and provide ongoing support where needed. Adhere to safeguarding procedures and raise concerns appropriately when working with children, families, and vulnerable adults. Promote equality, diversity, and inclusion in all aspects of your work. Community and Partnership Development Build strong links with grassroots organisations serving Black, Asian, Roma, refugee, and other minority communities, ensuring culturally sensitive support and inclusive referral pathways. Collaborate with schools, CAMHS, GPs, and voluntary sector partners to strengthen care pathways for children, young people, and families. Support the provision of early intervention for young people at risk of school exclusion, poor mental health, or antisocial behaviour. Work to improve access to NHS and community services for underrepresented and marginalised groups. Contribute to reducing stigma and breaking down barriers to engagement within diverse communities. Strengthen sustainable partnerships between health, education, and community organisations to improve service coordination. Contribute to ongoing service evaluation, supporting continuous improvement and demonstrating measurable impact. Person Specification Essential: GCSEs (or equivalent) in English and Maths, willingness to undertake relevant training as required for the role. Experience engaging with high-risk young people, with strong awareness of how community resources, early intervention, and social prescribing can reduce harm and improve outcomes Experience working with individuals with long-term conditions, families, or children in a support, advocacy, or community role. Understanding of the social determinants of health and the principles of social prescribing. Excellent communication and interpersonal skills, able to build rapport and trust with diverse individuals. Ability to work autonomously and as part of a multidisciplinary team. Strong organisational skills and ability to manage a caseload. IT literacy, including use of record-keeping systems. Knowledge of safeguarding procedures and ability to identify and escalate concerns appropriately. Desirable: Relevant training or certification in health, social care, youth work, or community development (e.g. NVQ Level 3 or 4). Experience working in a primary care, community health, or voluntary sector setting. Knowledge of local community services and support available to children, families, and adults with long-term conditions. Training in motivational interviewing, trauma-informed approaches, or similar frameworks. Additional Information The role may require travel across the local area or to community venues; a valid UK driving licence and access to a vehicle is desirable. The post holder will be required to undergo an enhanced DBS check prior to employment.
