Please refer to the attached job description and person specification for full details of responsibilities The nursing role is to offer specialist support/ interventions to service users with complex needs and/or behaviours that challenge to prevent admission to hospital or to out of area placements. To Participate in Care Treatment Reviews(CTR) for Service Users in hospital out of area or locally in assessment and treatment services or for individuals in the community. To complete the dynamic Support Tool so that service users can re rated on the Dynamic Support Register (DSR) to show levels of support needed on rage a rated system. To regularly monitor and review medication changes with psychiatry. To ensure that clinical practice is evidence based and consistent with relevant NICE guidelines and Trust policies and procedure
