JOB TITLE: Advanced Nurse Practitioner SALARY:Competitive dependent on experience. HOURS Full Time 37.5hours (Monday-Friday 8:00-6:00pm) RESPONSIBLE TO: The Partners, The Practice Manager and Nurse Manager Duties & Responsibilities You will be an experienced autonomous practitioner in primary care, with excellent communication and decision-making skills. You will be willing to be an important part of our close-knit supportive team. You will have strong minor illness expertise but also be able to manage complex and long-term conditions. Registered Nurse with NMC registration. You will be an independent prescriber and preferably hold, or be working towards, a Masters qualification. Experience working as an Advanced Nurse Practitioner within General Practice would be advantageous. Familiarity with SystmOne or Clarity Teamnet is beneficial but full training would be given. Manage on the day appointments alongside the Duty Doctor. Autonomously assess, examine, diagnose, plan, implement and evaluate treatment for patients. Manage acute and chronic conditions, including undifferentiated presentations, referring to appropriate clinicians when needed. Request, interpret and act upon diagnostic investigations such as blood tests, microbiology results or imaging, with appropriate senior clinician support where needed. Prescribe safely and effectively in line with local formularies. Support patients in adopting health promotion strategies, lifestyle changes and self-management. Communicate clearly and compassionately, ensuring patients understand options and are able to provide informed consent. Accurately maintain electronic and written records in accordance with practice policies and professional standards. Professional Standards Work within the NMC code of practice. Work collaboratively with nursing, medical, and administrative teams, attending relevant team meetings. Support and participate in clinical audits and service evaluation when required. Contribute to the development of clinical policies, pathways and practice improvements. Report complaints, incidents and near misses, contributing to risk reduction and organisational learning. Maintain ongoing professional development and engage in clinical supervision and reflective practice. Proactively identify learning needs and participate in training to ensure safe clinical practice. Provide clinical support to the development of colleagues including engagement with medical students. Education & Development Provide clinical support to the development of colleagues. Identify learning needs and participate in training related to safe clinical practice and equipment use. Contribute to staff development to ensure competence and high standards of care. Participate in research, clinical supervision and audit to continuously improve outcomes. Empower patients and families in self-management and understanding of their health conditions. Maintain professional development in line with NMC requirements and national guidance. Audit & Quality Improvement Support and participate in clinical audits and service evaluation when required. Apply evidence-based practice and evaluate outcomes to improve clinical standards. Assist in developing and implementing quality assurance processes, protocols and practice policies. Report complaints, incidents and near misses, contributing to risk reduction and organisational learning. Promote critical appraisal and evidence-informed practice within the team. Resource Management Human Resources Organise workload effectively to meet patient needs. Contribute ideas and feedback that enhance service quality and operational efficiency. Physical Resources Use equipment and clinical resources safely and responsibly. Support staff development to ensure competent and safe use of equipment. Freedom to Act Work autonomously across clinical boundaries to manage a caseload of undifferentiated minor illness or injury. Prioritise, organise and deliver work within the framework of practice policies, procedures and clinical guidelines. Maintain networks of support for yourself and the multidisciplinary team. Stay proficient in emergency resuscitation techniques. Health, Safety & Infection Control Take reasonable care of personal and others safety at work. Adhere to the practices infection prevention and control policies, including hand hygiene protocols. Follow relevant legislation and guidance (e.g., Health Act 2006, Health & Social Care Act 2008). Risk Management Develop, promote and implement effective risk management systems within areas of responsibility. Act to minimise risk to yourself, others and the practice where practicable. Identify and report hazards in line with practice policies and take immediate mitigating action when possible. Records Management, Confidentiality & Information Governance Create and manage patient information according to practice policies and legal requirements. Maintain confidentiality of patient and staff data, following Data Protection legislation and Caldicott principles. Ensure information is accurate, used appropriately, retained only when necessary, and kept secure. Patient & Public Involvement Support patient involvement in care planning, service development and improvement. Assist with initiatives such as the Patient Participation Group. Work independently and undertake additional duties within the scope of the role as required.
