This role is for a Band 7 Occupational Therapist, who will be based in an integrated Urgent Community Response Service based in Beaumont House at Mile End Hospital. The Band 7 Occupational Therapist will provide a high-quality, specialist service to adults and older people with a wide range of disabilities and complex needs, including chronic illness and long-term conditions. This community-based role sits within an integrated multidisciplinary Community Health Service and focuses on promoting independence, preventing hospital admissions, and supporting timely discharges through client-centred, goal-oriented interventions. As part of Role, the occupational therapist will collaborate with physiotherapists, nurses, social workers, GPs, and other professionals to deliver coordinated care that reduces unplanned hospital admissions and facilitates safe, supported discharges. The post holder will be expected to demonstrate strong clinical reasoning, leadership, and communication skills, and to act as a role model for best practice within the team.
