Crynodeb
It is expected that the post holder: Applies evidence-based care to support physical well-being utilising Learning Disability specific and mainstream health targets. Can assess and manage complex clinical issues in relation to service users;Can advise and collaborate with carers; professionals; other agencies to around complex clinical interventions and outcomes. Assesses, implements and critically evaluates intervention outcomes to maintainand improve the physical well-being of service users. Involves otheragencies/services as appropriate to meet need. Liaises with multi-disciplinary colleagues and other professionals / agencies toaddress issues that create barriers to social well-being and reports on unmet needsand obstacles to senior colleagues. Develops training for staff, users and carers around health promotion. Facilitate service users and carers in overcoming barriers to accessing primary careand other NHS services.
