43.75 average hours per week on rolling rota Week 1: 3 days Week2: 4 days

Shifts: 07:30 - 20:00

Main Responsibilities of the Support Worker:

Offer advice, information, and support to clients, including referrals for aftercare focused on maintaining an abstinent lifestyle.
Assist Therapists with risk management plans and reviews.
Ensure clients follow program rules, supporting the manager in investigating breaches, including substance testing.
Conduct intakes, assessments, and discharges efficiently.
Keep accurate client records and maintain databases.
Facilitate transfers between agencies and services for clients.
Support clients with medication reminders, conduct audits, and maintain accurate records.
Work with clients to create recovery plans that are realistic and achievable.
Help clients attend external appointments in line with their care plans, providing transport support or accompanying them as needed.
Assist with day-to-day operations, helping clients manage practical aspects of daily living such as life skills, housing, and benefits.

Strong organisational and time management skills.
Clear verbal and written communication at all levels.
Ability to work independently and as part of a team.
Proficiency in Word, Outlook, and Excel.
Excellent listening skills.
Ability to work safely and effectively on your own.
Strong negotiation and boundary-setting skills.
Ability to create and review risk management plans.
Empathy and a non-judgemental, calm approach.
Capacity to handle high-pressure situations.
Great customer service skills.
