Review of the Home Treatment Team case load in the multi- disciplinary team setting to ensure efficient and excellent care is delivered by the service, reducing the average patient time on the caseload, preventing hospital admissions and helping to form effective and lasting care plans enabling patients in crisis to be returned to their community teams safely.
Weekly screening of inpatient wards and assessment of patient suitability for early discharge under the Home Treatment Team, reducing average length of inpatient admission, creating bed space and reducing out-of-county admissions and place of safety re-purposing.
Bi-weeklymedical reviews of patients under the care of the Home Treatment Team, helping to update care plans, formulate and consolidate diagnosis and treatment.
Due to the nature of our local geography with long distances between homes, there will be a mixture of home visits and some scheduled clinics where patients in crisis are invited to attend outpatients clinic for review.
In their supporting professional activities time, the consultant may choose to help develop the county crisis service, and other aspects of emergency mental health care in Cornwall
